Chantel Anderson, DNP, PMHNP-BC, FNP-BC, returned to Pulaski Memorial Hospital’s Medical and Surgical Group in 2026 after becoming Board Certified as a Psychiatric Mental Health Nurse Practitioner.
As a Family Nurse Practitioner, Chantel can treat a wide range of acute and chronic medical conditions in patients of all ages, as well as provide well visits for vaccinations, CDLs, and sports physicals. As a Psychiatric Mental Health Nurse Practitioner, she is also able to see individuals with mental health conditions and substance use disorders.
Chantel is welcoming new patients at our Francesville Family Medicine Clinic location.
