Community Health Needs Assessment

Every three years the hospital completes a Community Health Needs Assessment to identify and prioritize the community's health concerns. PMH is asking for the public's input to help us meet those healthcare needs.

We have partnered with the Indiana Rural Hospital Association (IRHA) to develop a survey for the members of our community to complete. Paper copies are available at the admissions desk in the hospital, as well as at our four (4) health clinics location in Pulaski County and North Judson. For those wishing to complete the survey electronically, please click on the link below.

We encourage anyone eighteen (18) years or older to complete the survey. Your participation will assist us in achieving a thorough review of the health status and the unmet health needs in our community.
