Every three years, the Hospital completes a Community Health Needs Assessment to identify and prioritize the community's health concerns.
This year, we have once again partnered with the Indiana Rural Hospital Association (IRHA) to help us complete the assessment. Below is the link to the 2022 survey. Everyone 18 and older is invited to complete the survey and provide feedback on the health needs of our communities. The survey is open until June 19th. Once complete, IRHA will submit a final report to PMH, and those results will be publicly available on this page.
