Rehabilitation Services


The Rehabilitation Services department is concerned with the application of goal-oriented activities in the evaluation, problem identification, and treatment of persons whose function is impaired by physical, sensory, cognitive, developmental, and communicative impairments. Services include: Physical, Occupational, and Speech Therapy.

Care Settings
Rehabilitation Services are provided in a variety of patient care settings at Pulaski Memorial including:
  • Acute Care
  • Skilled Care (Swing Bed) program
  • Outpatient Services
  • Home Health Care

Request for Services
Services must be ordered by a qualifying health care provider such as a physician, nurse practitioner, podiatrist, chiropractor, dentist, or psychologist. A prescription for services (physical, occupational, speech therapy) must be presented along with an appropriate rehabilitation diagnosis in order for services to be rendered. Hours of Operation
8:00 a.m. through 5:30 p.m. Monday-Thursday
8:00 a.m. through 4:30 p.m. Friday
Weekends PRN coverage in Physical Therapy only

Physical Therapy
Physical therapists help people recover function after loss of strength, motion, endurance, balance, coordination, and sensation. Conditions treated include orthopedic conditions such as low back and neck pain, post-surgical rehabilitation; joint and soft tissue injuries such as fractures and dislocations; neurologic conditions such as stroke, multiple sclerosis, or Parkinson's disease; connective tissue injuries such as wounds; cardiopulmonary and circulatory conditions such as congestive heart failure and chronic obstructive pulmonary disease; and workplace injuries including repetitive stress disorders and sports injuries.

Occupational Therapy
Occupational therapy focuses on enabling people to do the activities of daily living. The very word "occupation" means an activity that "occupies" our time. A person has the responsibility of performing self-care activities (dressing, bathing, feeding) and maximizing independence and function in home, work, and community environments. All of these things are occupations and participating in them is vital to maintaining overall health and wellness. Occupational therapists can help restore function through environmental modification/adaptation, work simplification, improving motor skills of the upper extremities, bracing/splinting the hands and elbows for joint protection and pain reduction, and cognitive/sensory integration techniques.

Speech Therapy
Speech Therapy involves the rehabilitation of speech, language, cognitive, and swallowing skills that may deteriorate as a result of a medical issue/injury, neurological condition, developmental delay, and various other conditions. Services for all age groups with communicative disorders including screening, speech assessments, language development, rehabilitation of verbal and written language, articulation, fluency, interactive communication, and cognition are offered. Modified barium swallows (cookie swallows) are also performed at PMH by a speech pathologist and a radiologist to assess the swallow mechanism for conditions such as dysphagia, allowing recommendations for treatment, diet modification, and decreasing the risk of aspiration.
